Taking new participants now

Nursing care at home, led by a nurse practitioner.

Disability-related health supports for NDIS participants across Perth, planned around your disability and delivered where you live.

Andriette du Plessis, nurse practitioner

Who we support

NDIS participants whose disability brings complex health needs. For example, people living with an acquired brain injury, cerebral palsy, motor neurone disease, multiple sclerosis, a spinal cord injury, younger onset dementia, or an intellectual disability with complex health needs.

We work with NDIA-managed, plan-managed and self-managed participants.

Health supports we provide

Named the way the NDIA names them, so they match your plan.

  • Dysphagia supports

Swallowing plans, texture-modified meals and safe mealtime support.

  • Nutrition and enteral feeding

PEG and tube feeding, feeding plans and equipment.

  • Continence supports

Continence assessments, catheter care and complex bowel care.

  • Wound and pressure care

Preventing and managing pressure injuries and wounds related to your disability.

  • Respiratory supports

Tracheostomy care, ventilation support and respiratory plans.

  • Seizure supports

Seizure management plans, emergency medication and training.

  • Diabetes management

Blood glucose monitoring, insulin support and plans your team can follow.

  • Medication management

Medication plans, reviews and safe administration by your support team.

We train the people around you

Support workers, family and friends learn to deliver your health supports safely, and we stay responsible for the care we hand over to them.

Personal care and help getting to appointments can be arranged as part of your care plan.

How it works

  1. Free first consult

By phone or Zoom. We talk through your needs and your plan.

  2. Assessment at home

Our nurse practitioner assesses your health needs where you live.

  3. Your health care plan

Each support linked to your disability and goals, written so your team can follow it.

  4. Care and training

Nursing visits as you need them, and training for the people who support you every day.

  5. Reviews and reports

Regular reviews, and the reports you need at plan reassessment.

What is a nurse practitioner?

A nurse practitioner is a registered nurse with a master’s degree and endorsement from the Nursing and Midwifery Board of Australia to practise at an advanced level. Nurse practitioners assess and diagnose, order tests, prescribe and refer, within their scope of practice.

For you, that means fewer gaps: one experienced clinician who understands your health, your disability and your plan.

Primary care with your nurse practitioner, through Medicare

Some health needs aren’t NDIS supports: a new illness, routine prescriptions, or pathology and imaging requests. Our nurse practitioner can see you for these in a Medicare consultation, bulk-billed or with a gap fee, depending on the consultation, kept completely separate from your NDIS plan.

Support at appointments

We can come with you to GP, specialist or hospital appointments, bring your health care plan, and make sure you’re heard.

Your care team

Led by our nurse practitioner, with registered nurses and trained support workers as your needs require.
