Gesond Pty Ltd (ABN 43 619 945 210) provides nursing and related health supports to NDIS participants. We collect personal information — including health information — only so we can provide safe care and meet our obligations as a registered NDIS provider.

Contact details; NDIS plan details; health information relevant to your care; referral information from you or, with your consent, from your support coordinator, provider or health service.

How we collect it

Directly from you, through the forms on this website, and from people you have authorised to share information with us.

How we use it

To assess and deliver your supports, write care plans and reports, claim against your NDIS plan or Medicare where that applies, train the people who support you (with your consent), and meet the NDIS Practice Standards and other legal requirements.

Storage and security

Your information is stored securely with access limited to the people involved in your care and our administration.

Disclosure

We share your information only with your consent, with people involved in your care, or where the law requires it. We never sell personal information.

Website forms

Referral and contact forms send your information only to Gesond’s own email.
